Connection Application Form

MainPower requires the Connection Application Form be completed and returned to MainPower in all instances where a connection to the North Canterbury distribution system is required in order to deliver electricity to a consumer. The form can be completed by an electrician, builder or customer.

There are two ways that you can complete the form;

Printed
	You will need to return the manually completed form to MainPower via fax, post, email or hand delivery with plans attached   For a printable version of the Connection Application Form please on the image to the left.
Online
	You can also forward the plans electronically attached to the form.   To complete your application online, refer to the bottom of this page. Please do not print this form
